Coming soon, Wesleyan Health & Rehabilitation Center is pleased to announce the opening of our brand-new, innovative in-house dialysis unit in partnership with Concerto Renal Services, which is designed to revolutionize the way we provide care to our patients. We are committed to ensuring that every individual receives the highest quality of care in a comfortable and convenient environment.
We understand that traveling to a dialysis clinic can be disruptive, time-consuming, and exhausting for dialysis patients and their loved ones. Our rehabilitation and skilled nursing facility with in-house dialysis now offers many benefits to those receiving treatment for kidney failure and different stages of renal disease, families, and hospital partners, including:
- Eliminating the need and risks of transporting patients with higher risks of infection
- Enhancing quality of life, with more time for rehabilitation and skilled nursing facility (SNF) activities
- Reducing the transmission and spread of contagious diseases
- Providing a comfortable, homelike environment for dialysis treatments
- Promoting the patient’s entire care team to collaborate, resulting in more favorable clinical outcomes
- Reducing the patient’s length of stay in the hospital with an efficient discharge process into the hemodialysis unit
- Allowing the patient’s choice of receiving all the long-term care they need, including dialysis and therapies, within their residence
- Allowing real-time collaboration with the SNF clinical teams to address patient issues in-house – rather than immediate transfer to an emergency department
Concerto’s on-site, three-day dialysis program at Wesleyan requires less treatment time than daily dialysis and eliminates hours of travel time per week. The three-day model also provides consistency in treatments for our patients when they return to the community. Three-day dialysis requires less frequent use of a patient’s vascular access site, significantly lowering the potential for infection and complications.
How Does Our In-House Dialysis Process Work?
Each patient in the unit is assigned a dialysis care team consisting of experts with the following responsibilities:
NEPHROLOGIST
- Specializes in kidney health and is the leader of the kidney care team
- Assesses the patient’s dialysis needs and prescribes medications
- Works closely with nurses, technicians, dietitians, and social workers
NURSE
- Schedules the patient’s treatments, carries out the prescribed plan, and oversees patient treatments while working with the facility care team
RENAL SOCIAL WORKER
- Provides support for the patient and the family to manage lifestyle and emotional adjustments
RENAL DIETITIAN
- Helps patients and their caregivers manage their unique dietary needs
PATIENT CARE TECHNICIAN
- Facilitates a safe and comfortable dialysis treatment through their expertise in operating dialysis equipment
